Swing bed program

At Prosser Memorial Health, patients in our Transitional Swing Bed Care Program receive individualized in-hospital care and physical rehabilitation, with the goal of helping them reach their highest level of function with their activities of daily living when they're ready to go home.

Bridging the gap from hospital to home

Our swing bed program provides transitional care to patients in a hospital setting. It is designed for patients who are discharged from an acute care facility but who need temporary additional supportive care that can't be provided at home.

Referrals to our swing bed program can come from several sources, including:

  • Your physician
  • Your hospital discharge planner or case manager
  • Insurance case managers

​Through a combination of rehabilitative therapies and medical and nursing care, swing bed patients gain the function they need to achieve their maximum level of independence in a caring, supportive environment.

Our swing bed program provides all the medical services you may need during your stay, from respiratory therapy to IV therapy, to podiatry and wound care. Medical imaging and laboratory services are also available.

Our skilled and experienced swing bed team

A team of highly trained providers will be involved in your care during your stay with us. These include:

Hospitalists. A physician is available to manage your care 24 hours a day. In addition, a physician is on-site and available to provide immediate care in the event of an emergency.

Nurses. They assist with individualized daily care, as directed by the hospitalist, and ensure a smooth transition to home.

A social worker. Our social worker offers psychological support and connections to services you may need after you're discharged, like rides to follow-up appointments.

Dietitian. A licensed dietitian will provide nutritional recommendations to help with your healing and health.

Rehab specialists. Physical, occupational and speech therapists may work with you to help you reach your physical goals.

Wound specialist. A certified wound care nurse will assess and work with the hospitalist to determine a treatment plan for optimal wound healing.

Activities and amenities

You will get stronger as your rehabilitation progresses. Once your doctor or nurse determines that you're strong enough, you can take advantage of some of our activities and amenities outside our department. These include our tranquil courtyard; gazebo and healing garden; and our communal bookshelf, cafeteria, café, gift shop and gym.

Going home

Our swing bed care team will monitor and evaluate your progress and goals. When they feel you're ready to go home, they'll make sure you have a safe discharge. If needed, our team will refer you to other services like home health, physical therapy and occupational therapy.
